How to Join:
To qualify for membership, you must be 18 years old and abovePresent 1 valid Identification CardPresent your P100 proof of payment to any of our Baby Specialist.Fill-up the personal information form and submit it to any Baby Company Personnel at any Baby Company store to claim your Mom Card.
Your Membership:
Membership duration is valid for three ( 3 ) years from date of issuance.To report a lost or block card, call the Baby Company Mom Card hotline, log on to Baby Company website, or approach any of the Baby Company store personnel inside SM Department Store and Baby Company mall stores. Once card is blocked, it cannot be reactivated. Pay the corresponding replacement fee of P100 at any cashier counter assisted by a Baby Company sales personnel. Points from lost card may be transferred to a new replacement card provided the member filled up the previously issued Membership Application Form properly and completely for said lost card.Card Activation will take effect upon submission of your filled up membership form. You will receive a notification message via email or SMS that the card has been activatedIn case of defective or damaged card, go to any Baby Company Store to request for a card replacement. If reported within the 3 month warranty period, replacement is free provided that your card does not have any visible sign of deliberate physical abuse.
